Magnus Health Medical Forms Due
Valor partners with Magnus Health to provide a safe and secure way to collect and manage student records. By May you will receive a welcome email from Magnus Health providing you a link to their system and login credentials.

For your convenience, we will be collecting all necessary student forms for the upcoming school year through Magnus Health. The best part is that once you’ve completed and uploaded the documents, you will only need to update records each year. Another benefit is the parent customer service line available to assist with any questions about the Magnus website.

As soon as you receive your email from Magnus, we recommend that you get started on the process, as some of the forms require a physician signature.
